Pregnancy in High-Risk Pulmonary Embolism

One of the most common causes of maternal mortality in the Western world is pregnancy-associated high-risk pulmonary embolism, which results in hemodynamic instability and circulatory failure due to a significant thrombotic pulmonary blockage. The extremely difficult management of these dramatic conditions includes the necessity to immediately decide between pulmonary reperfusion therapy and hemodynamic replacement while taking both maternal and foetal risks into consideration. The relevance of risk stratification in pregnancy-related PE and the data supporting the use of thrombolysis, catheter-directed thrombectomy thrombolysis, surgical embolectomy, and extracorporeal membrane oxygenation are included in this study. The majority of documented cases of high-risk pregnancy-associated PE have been treated with thrombolysis, with good mother and foetal survival rates, and thrombolysis is recommended by despite the absence of comparison studies and strong evidence.
